Payment Options
We offer several payment options to help our patients cover their treatment costs. Please contact our billing department at 920-430-8120 for more information.

Insurance Coverage

Patients utilizing their health insurance coverage can check if OSMS is in-network by visiting our Insurance Coverage page or by calling the OSMS Business Office at 920-430-8120. As a reminder, we accept most insurance plans and typically a referral is not required for orthopedic, sports medicine, and pain management care.

Self-Pay Options

Pre-Service:

OSMS has partnered with MDSave for our self-pay patients!

What is MDSave:

  • A pre-service payment option for certain Orthopedic & Sports Medicine services and locations – shop below for available services.
    • For Rheumatologic, Pain Management & Spine self-pay options, please contact our OSMS Business Office.

Post-Service:
If you have a current balance at OSMS, please click the button below to make your payment via debit or credit card. If you are unable to pay your bill in full, please call our Business Office at 920-430-8120 for additional payment options.

    Choose which procedure or service you need done and with what eligible OSMS provider and eligible OSMS location you would like your care from.

  • Step 2: Make Payment

  • Step 3: Visit OSMS

    Schedule your procedure or service and then visit OSMS to receive your care. Please bring your voucher to prove payment.
